WMCHealth and MVP Health Care Launch Collaborative Care Partnership
BCW members, Westchester Medical Center Health Network (WMCHealth) and MVP Health Care have announced a new partnership designed to enhance care coordination and improve health outcomes for patients throughout the Hudson Valley.
The collaboration brings MVP Health Care’s clinical care managers directly into the care environment at Westchester Medical Center, where they will work alongside WMCHealth medical teams to support MVP members as they transition from the hospital to home. The initiative aims to create a more seamless patient experience by helping individuals navigate follow-up appointments, specialist referrals, and ongoing care after discharge.
By aligning clinical care plans with insurance benefits and support services, the partnership seeks to reduce gaps in care, improve communication among providers, and help prevent avoidable hospital readmissions.
“Patients are at the center of everything we do, and this unique partnership between a health care system and insurer will provide patients with highly coordinated care that is relentlessly focused on their long-term health and well-being beyond the hospital,” said Dr. David Lubarsky, President and CEO of WMCHealth.
Dr. Richard Dal Col, President of MVP Health Care, emphasized the importance of collaboration in shaping the future of health care, noting that stronger partnerships between providers and insurers can help create a more connected, efficient, and patient-centered experience.
The partnership reflects a growing focus across the health care industry on integrated care models that support patients beyond hospital walls and promote better long-term health outcomes.
