VNS Westchester Pilots Patient Outreach Call Program

VNSW is partnering with health technology company, CipherHealth, on an innovative pilot program aimed at reducing preventable hospital readmissions, improving patient health and better managing chronic illnesses among our home care patients. The “VNSW Care Call” is an interactive patient engagement platform that generates a series of calls to patients about their health status, creating a pathway for early health interventions.
